CME/CE

Only individuals who registered for the Palliative Care Conference before April 29, 2010 are eligible for CME/CE Credits.  The credits are available at no cost to registered participants who joined by webcast or at a facility.

Instructions for Participants who want CME/CE:

Each participant should only claim credit commensurate with the extent of participation in the activity. 

Participants must do three things to be eligible for CME/CE credits.

  1. Register for the conference on-line indicating that credit is wanted.
  2. Sign the Participant Sign-in Sheet for each day in attendance if attending at a facilty.
  3. Complete and return the four-page CME/CE Evaluation Form (the fourth page must be signed by the participant).  The forms are available on-line below - You must be logged in to Access the Forms.

If viewing the Symposium at a facility, a Site Facilitator will mail the Sign-in Sheet to ANTHC.  The participant can either return the CME/CE Evaluation Form to the Site Facilitator or mail or FAX it directly to Judith Muller at at the address or FAX below.  Failure to do any of the three above may result in credit not being given.

On Symposium Days:

If attending the symposium at a facility, each day, every participant must sign the Participant Sign-in Sheet.   CME/CE Evaluation Forms from each participant who is requesting CME/CE must be filled out.  Participants  must sign-in and complete an evaluation form to be eligible to receive education credits.   At the close of each day  those participants who are NOT returning on the next day must complete a CME/CE evaluation form and return them to the facilitator.

After the Symposium:

Completed CME/CE Evaluation Forms and  Participant Sign-in Sheets should be sent to the Alaska Native Tribal Health Consortium:

Participants who have met the requirements for CME/CE will receive a signed certification of CME/CE Credit in the mail.

Credits by Webcasting:

Sites without facilitators or individuals joining by webcasting must sign a conference sign-in sheet to certify their attendance at the webcast.  Those individuals should use the sign-in sheet available below.  In order to receive credit they must send the sign-in sheet along with the evaluation form to Judith Muller.

Accreditation:

In order to recieve credit, you must download, complete, and mail in all three CME/CE forms.
  

Physicians:

Alaska Native Medical Center is accredited as a sponsor and provider of continuing medical education for physicians by the Alaska State Medical Association.

ANMC designates this activity as meeting the criteria for one hour of AMA PRA Category 1 Credit™ for each hour of participation up to a maximum of 11 hour(s).  Each participant should only claim credit commensurate with the extent of participation in the activity. These hours may be used by physican assistants and social workers practicing in Alaska.

Nurses:

Alaska Native Medical Center is an approved provider of continuing nursing education by the Alaska Nurses Association, an accredited approver by the American Nurses Association Credentialing Center’s Commission on Education. 

ANMC designates this activity as meeting the criteria for one nursing contact hour credit for each hour of participation up to a maximum of 11 hour(s).  Each participant should only claim credit commensurate with the extent of participation in the activity.
